How to measure a resident's height and weight on an upright scale

1. Wash hands, greet the resident, and verify identification. 2. Explain what you are going to do. 3. Assist the resident to a standing position and stepping on to the scale, facing away from the sliding weights or monitor. 4. Lower the height rod until it rests gently on top of the resident's head. Record the height. 5. Help the resident turn to face the sliding weights or monitor. Measure and record the resident's weight. 6. Assist the resident from the scale to their chair or bed. Position for comfort. 7. Wash hands.
