1. Measure urine before discarding. 2. Liquid stool should be estimated, measured by using a hat over the toilet, or measured after the client uses a bedpan. 3. Vomit should be estimated if it can't be measured. 4. If the client has an N-G tube or wound connected to suction, measure the quantity for the shift. 5. If a draining wound is not connected to suction, record how many times the dressing was changed. 6. Record all output for the shift on the client's chart.
