How to observe a resident's mental status
1. Notice if the resident is alert, or difficult to waken or arouse. 2. Watch for signs of confusion, such as disorientation to time and place. 3. Immediately notify the nurse if the resident has sudden difficulty speaking, slurred speech, or inability to follow commands.
