What is included in an Incident Report?

Names: Person responsible for, or directly involved, as well as anyone who was affected by the incident. List of people who were witnesses. Date/time/location: Exact information must be documented. If the time is not known (Example: a client is found on the floor), record when the incident was discovered and the last known time the client was observed. Description: Record the facts of what happened, what actions were taken at the time of the event, or following the event.
