What is included in charting?
1. Vital signs, intake & output, activities; 2. Checklists, often with assessments and routine care; 3. Procedures and how the patient tolerated them. If there are any unusual observations or assessments, these can be charted in narrative notes. The CNA should follow the facility's policies on charting.
